Stress and Its Effects on Health Essay

Stress is the emotional strain or tension experienced by an individual due to a reaction toward various demanding and influential situations. The challenging or compelling situations are termed stressors. Stressors can be internal or external and include life changes such as losing a significant figure, low socioeconomic status, relationship problems, occupational challenges, and familial or environmental factors. An individual’s response to stressors influences the outcome of their life. Health is a state of complete social, emotional, and physical well-being and not merely the absence of disease. Stress is a common risk factor for negative health status secondary to negative adaptation and coping with the stressors. Stressors can create a strain on one’s physical, psychological and behavioral well-being, leading to lasting effects that are detrimental to one’s health.

Stress is associated with various physical health impacts on an individual. In an online cross-sectional survey by Keech et al. (2020) to determine the association between stress and the physical and psychological health of police officers, the findings illustrate that stress negatively impacts physical and psychological well-being. One hundred and thirty-four police officers were involved in the study (Keech et al., 2020). The findings demonstrate that stress resulted in various short and long-term physical effects that included increased heart rates, sweating, high blood pressure, and long-term development of the cardiac condition. In addition, stress resulted in the development of gastrointestinal disorders such as peptic ulcer and irritable bowel syndrome. Keech et al. (2020) note that stress’s associated physical health effects are explained by various mechanisms that include overstimulation of the sympathetic nervous system and the hypothalamic-pituitary-adrenocortical axis.

Overstimulation of the sympathetic nervous system results in increased sympathetic actions on the peripheral body organs leading to increased sweat production, heart rate, respiration rate, and urinary and bowel elimination. The study notes that chronic stress without positive adaptation measures results in the progressive development of hypertension, peptic ulcers, and irritable bowel syndrome as long-term effects (Keech et al., 2020). Within the gastrointestinal tract, chronic stress activity on the sympathetic nervous system results in increased parietal cell action. Overactivity of the parietal cells results in excessive gastric acid production, gradually eroding the mucosa, and ulceration occurs.

The effects of stress on the cardiovascular system are explained in a review by Kivimäki & Steptoe (2017) to determine the impact of stress on the development and progression of cardiovascular diseases. In the review, stress is identified to cause cardiovascular conditions secondary to the effects of sustained sympathetic action on heart contractility and peripheral vascular resistance (Kivimäki & Steptoe, 2017). The sympathetic nervous system contributes to normal heart and blood vessel contractility. However, when the system is overstimulated, a surge in contractility above the normal limits ensues, leading to the progressive development of heart conditions.

Psychological well-being incorporates a positive mental health status evidenced by an individual’s satisfaction with life, happiness, rational thinking and decision-making, and positive mood patterns. Stress has been associated with alterations in an individual’s psychological wellness. An explanation for alteration in an individual’s psychological well-being secondary to stress is negative adaptation. Keech et al. (2020) note that an individual’s response to a stressor determines whether stress results in positive or negative effects. In the online cross-sectional survey by Keech et al. (2020), the findings illustrate that pressure resulted in the development of anxiety, depression, and bipolar disorders as long-term effects among the participants. Exposure to stressful situations resulted in progressively developing anxiety among the individual secondary to persistent worry over the issue. The anxiety results in other physical manifestations, including increased heart rate, palpitations, sweating, and altered mobility. Depression and bipolar conditions were also associated with chronic stress secondary to the impacts of stress on neurotransmitter function and nerves.

Similar findings are noted in a cross-sectional study by Zhang et al. (2020) to compare the prevalence and severity of stress-associated mental health symptoms, including anxiety, depression, and insomnia among healthcare workers during the COVID pandemic. Five hundred and twenty-four healthcare workers were involved in the study. The study findings illustrate that 31.3% of the participants developed depression secondary to the stressful working environment, 41.2% reported anxiety, and 39.3% reported sleep disturbances (Zhang et al., 2020). The scientific explanation for the relationship between stress and depression was attributed to the effects of stressful periods on neurotransmitter homeostasis. Chronic stress results in the altered regulation of neurotransmitters in the central nervous system. Alterations in serotonin, norepinephrine, and dopamine resulted in the progressive development of depression and anxiety. Sleep disturbances reported by the participants are attributed to alterations in cortisol hormone homeostasis secondary to overstimulation of the hypothalamic-pituitary-adrenocortical axis.

Stressful situations can also lead to alterations in the behavioral patterns of an individual. The most common behavioral effects secondary to stress include the development of eating disorders, altered sleeping patterns, impaired concentration, and drug abuse especially alcohol. Alterations in sleep and eating patterns are linked to stress’s effects on the hypothalamic-pituitary-adrenocortical axis (HPA). Exposure to stressful events leads to increased activation of the HPA axis with a net effect of increased catecholamine production (adrenaline and noradrenaline) (Moustafa et al., 2018). Increased adrenaline and noradrenaline production results in dysregulation in the eating and sleeping patterns. Sustained high levels of cortisol results in difficulty falling asleep and increased metabolic processes. The biological clock regulates the typical sleeping pattern that relies on producing the sleep hormone melatonin. Melatonin production by the pineal gland is regulated indirectly by the concentration of serum cortisol levels and directly by light perception. Imbalances in the serum concentration cycle secondary to stress results in imbalanced melatonin production and concentration with a net effect of sleeping difficulties.

The emotional strain caused by stress increases the risk of alcohol and other illicit drug use and dependence. Moustafa et al. (2018) conducted an integrative literature review to determine the relationship between childhood trauma, early-life stress, alcohol and drug use, addiction, and abuse. The review findings illustrate that stress increases the risk of alcohol and drug use, addiction, and abuse among the victims. An explanation for the increased risk is the individuals’ lack of identification and implementation of effective coping strategies (Moustafa et al., 2018). Lack of effective coping strategies results in maladaptive measures such as illicit drug use and alcohol consumption. Extensive use of the maladaptive measures results in progressive addiction and drug abuse among individuals with an increased predisposition to other health effects. Alcohol consumption and other illicit drug use over time increase the risk of developing cardiac, respiratory, and liver conditions.

Stress is the emotional strain or tension experienced by an individual due to a reaction toward various demanding and influential situations. Individual response to stressors influences their health. Maladaptive response to stress results in various physical, psychological, and behavioral negative effects. Negative effects of stress on physical health include increased heart rates, sweating, high blood pressure, and long-term development of the cardiac condition. Psychological effects include the development of anxiety, depression, and bipolar disorders. The behavioral effects of stress on an individual include the development of eating disorders, altered sleeping patterns, impaired concentration, and abuse of alcohol and other drugs. Based on the research findings, it is essential for healthcare providers to identify strategic measures and health initiatives to educate and sensitize the community members on effective stress management approaches in all settings to aid in combating the health effects.

Reflective Practice and Stress: Helpful, Harmful or Uninfluential in Critical Thinking

Remember the first time you watched a master clinician work through a diagnostic dilemma or clinical unknown case and easily arrive at the correct diagnosis? That person most likely seemed calm with little stress, asked the right questions at the right time, and often reflected on the information they gleaned from the case. If the clinician described how they arrived at their diagnosis, you were probably in awe at how they solved the case using their critical thinking skills. While critical thinking is essential for effective patient care, this highly complex process is not easily taught or measured.

Critical thinking can be seen as having two components: 1) a set of information and belief generating and processing skills, and 2) the habit, based on intellectual commitment, of using those skills to guide behavior. 1 Thus, while possessing medical knowledge is a necessary part of critical thinking, many factors play a role in the process, such as data gathering skills, clinical context, patient preferences, personal reactions to stress and reflective practice. Research demonstrates that some stress and thoughtful reflection are beneficial to the thinking process, 2 – 4 yet there is no consensus on how to positively manage stress or use reflection in daily clinical care. Two articles 5 , 6 in this issue of JGIM explore the areas of stress and reflection on critical thinking among trainees.

The study by Pottier and colleagues 5 explored the role of stressors on a medical student’s history taking, physical examination, and clinical reasoning. The authors performed a study of medical students who participated in two scenarios with standardized patients with familiar disease processes. Students conducted a history and physical exam and were asked to generate a diagnosis with a differential based on the clinical data obtained during the scenario. During their patient encounters, students were presented with two stressors: extrinsic and intrinsic. Extrinsic stressors were manifested by the patient’s cooperativeness and mood such that the patient was either: a) aggressive with negativity and lacked confidence in the student’s competence; or b) pleasant without challenges to the student. Intrinsic stressors were defined as stressful components integral to the encounter based on clinical presentation, and were exhibited by a patient with either high clinical severity (dyspnea from an acute pulmonary embolism) or low clinical severity (acute abdominal pain in a relatively healthy patient). Students were randomized to two of four clinical scenarios with a balance of high and low clinical severity in an aggressive or pleasant patient. Subjective stress and anxiety responses were assessed before and after each experience, as were completeness in physical examination, communication skills, diagnostic accuracy and differential diagnosis argumentation. The authors found that each type of stress had a different effect on a student’s anxiety and clinical performance. Results indicated that extrinsic stress increased personal anxiety, but led to more accurately performing the clinical examination and increased interpersonal skills. Intrinsic stress did not change the students’ anxiety, but demonstrated increases in diagnostic accuracy and differential diagnosis scores. The perceived cognitive difficulty of the task was a strong factor in most of the stress and performance variables. When the clinical scenario had high extrinsic and intrinsic stress, students no longer showed improved clinical examination or communication skills. Interestingly, the initial exposure in the first case to extrinsic and intrinsic stress did not influence the outcomes of the second case.

In their work on reflection in decision making, Monteiro and colleagues 6 aimed to examine how a resident’s clinical experience and self-directed decisions to reflect on their diagnosis affected the accuracy of their decisions. Using a randomized mixed design, 47 medical interns and residents were presented with 16 computerized cases where they were given a patient’s primary complaint, medical history, a representative photograph, diagnostic tests and images. On their first review of the case (first pass), trainees were asked to arrive at a diagnosis as fast as they could (presumably their response in the automatic or intuitive, System 1 mode of thinking). On the second pass, residents were then randomly assigned to review either the full case again or a brief description of the case and asked to reflect on their decision and revise their diagnosis (encouraging System 2 mode analytic thinking). Residents arrived at the correct diagnosis in 58-64 % of cases with no significant differences in diagnostic accuracy among the three resident training levels. Only 8 % of diagnoses were revised with minimally significant changes in diagnostic accuracy from the first pass to the second. Having access to the full case on the second pass resulted in a higher rate of diagnosis revision, but did not affect diagnostic accuracy.

Both aforementioned studies explored factors that influence critical thinking, but limitations of these studies should be considered. First, both studies involved trainees and not practicing clinicians with more extensive medical expertise. For the medical students, their limited medical knowledge and clinical experience may have been most influential on their stress and critical thinking, as their perceived cognitive difficulty of the task itself was the greatest predictor in their performance. In the case of the medicine residents, these trainees also appeared to be limited in their medical knowledge, given their low diagnostic accuracy in the study (58-64 % correct diagnoses). Thus, no amount of analytic thinking without access to outside resources may have helped improve their diagnostic thinking. Second, both studies used simulation and not patient encounters. While it is unclear how these results may be extrapolated to performance in actual clinical practice, simulation is successfully used in both teaching and evaluation in lieu of patient interactions.

While these studies have limitations, the topics of stress and reflection in critical thinking are of high interest to clinicians. Similar to what occurs in high performing athletes, the medical literature has suggested that “good” stress can positively influence performance compared to “bad” or no stress. 2 , 3 Thus, just as athletes spend extensive amounts of time training to overcome obstacles and stress, perhaps a part of medical education should involve ways to teach students and residents how to lessen negative stress and improve positive stress. Educating trainees about personal wellness is currently being addressed by the Accreditation Council on Graduate Medical Education (ACGME), which requires each residency program to provide resident and faculty education on trainee alertness and fatigue mitigation. 7 Understanding and educating about stress and its impact on clinical care may be as critical as knowledge of fatigue and its effects on personal and professional well-being. As shown in the Pottier article, having good intrinsic stress increased students’ clinical reasoning, while extrinsic stress helped students focus on communication skills. These positive stress effects influenced critical thinking and the outcomes of the clinical encounter. Finding ways in the curriculum to educate medical students about the important connection between communication and critical thinking can be done successfully, as was seen in a preclinical course at Johns Hopkins University School of Medicine. 8 This course found that teaching medical students the integration of communication and clinical reasoning was a viable method to improve therapeutic relationships with patients while reducing the potential for medical error and harm. Perhaps more programs should explore the effects of teaching these skills together as a way to lessen bad stress and improve good stress.

Reflective practice has been described as an essential part of professional competence. 9 It is often considered to be an iterative process that occurs in more senior clinicians, especially when faced with a diagnostically challenging patient. 10 While a divergence of opinion exists regarding whether or not reflective practice is useful or can be taught, educators should consider ways to best foster reflective practice early in clinical training, as it is deliberate practice and reflection that pushes us to be better clinicians and teachers.

Whether reflective practice and stress are helpful, harmful or uninfluential on critical thinking, we should all remember that our goal in medicine is to establish effective therapeutic relationships with patients while reducing diagnostic errors. Recognizing what a patient brings to an encounter with respect to their biological, cultural and psychosocial background is critical in decision making. Understanding how we each react to stressors in a clinical context and how this may negatively or positively influence our medical decision making is as important as understanding our strengths and limitations in our medical knowledge. In addition, reflection in practice, especially when taking the time to gather more information, may be critical to avoiding diagnostic errors and premature closure.

Taking Time for Reflection: Managing Stress With Positive Thinking

Holidays are usually viewed as a joyous time. However, for some families, holidays can be stressful, as finances may be tight, family communication could be lacking or decreased daylight in winters may diminish mood and motivation. Changes in a swine caregiver’s mood, attitude, and behaviors can also begin to impact pigs in their care. Taking time to reflect involves slowing down and focusing on thoughts, feelings and behaviors. In the process, you may remember challenging times, including tough decisions you may have been forced to make about your swine operation. These reflections can help you critically evaluate strategies for the future and recall all the blessings that occurred. Each new day provides blank pages to fill with positive thoughts.

Both farmer welfare and animal welfare are important to the success and sustainability of the livestock industries. Research investigating dairy farmer welfare and animal welfare indicators showed connections between a farmer’s job satisfaction, overall personal welfare and stress level with an animal welfare index. 1 Research of swine caregivers also reveals relationships between a person’s attitudinal and behavioral profile towards pigs with the levels of fear sows show towards humans, which can also negatively impact reproductive performance measures. 2

Recognizing Stress

Knowing that stress impacts each person differently, it can be helpful to monitor your stress signs and symptoms. Where do you feel stress in your body? Do you experience physical signs, such as fatigue, headaches, aches and pains or digestive troubles? Maybe you have other stress signs that are emotional (moodiness, irritability, anger, loneliness, anxiousness, panic), cognitive (forgetful, pessimism, negative thinking, constant worrying) or behavioral (eating too much or too little, concerns with sleep, use of alcohol, drugs or cigarettes). Acknowledging and addressing these signs and symptoms is critical in maintaining a safe workplace for yourself and a good environment for your animals. Furthermore, handler behavior and attitude are shown to impact animal behavior (e.g. fear responses), production (e.g. growth and pigs born per sow, per year) and reproduction. 3

It is important to recognize when you are stressed and determine the triggers that elevate your stress. Reflect upon all aspects of your job. Which parts of your job cause you stress? What stressors are you attempting to manage, and which ones can you control? When we worry about stressors outside of our control, our thoughts can become negative and feel difficult to overcome. Handlers should monitor negative attitudes, as sows especially are sensitive and may become more belligerent and difficult to move as handlers get frustrated. Swine caregivers responsible for euthanasia have a high-stress job. Continued research is shedding light on perceptions of euthanasia and its impacts on the person in the workplace and the need for euthanasia resources to address mental well-being, as euthanasia can be a moral stress. 4,5

If stress is not managed, mental health can decline and become concerning. Producers experience high levels of psychological distress, depressive symptoms and anxiety symptoms. 6 Of greater concern, suicide rates among those who work in agriculture are estimated to be 1.5 times higher than those who work in other occupations. 7 Overall, chronic stress has taken a toll on farm and ranch families.

Managing Stress Effectively

As you think about the next six to 12 months, determine what you can control. For example, producers can be proactive and may feel more in control by creating and/or updating written emergency action plans and testing equipment for potential natural disasters (e.g. blizzards, floods) or facility issues (e.g. power outages). The positive thinking connected with being prepared contributes to optimism and can be a key aspect of effective stress management. Expecting the best in situations can be extremely challenging if you tend to be pessimistic and look and prepare for the worst. However, positive thinking has many health benefits, including lower rates of depression and psychological distress, improved physical health and better coping skills.

When you have been on a downward spiral of negative thoughts, the practice of positive thinking can seem daunting. Setting small goals is beneficial. Identify an area of your life in which you have negative thoughts and attempt to approach that area in a more positive way. Notice and monitor your thoughts related to that area. When negative thoughts sneak in, try to put a positive spin on the thought. Realize that market prices are out of your control, so stressing over them is fruitless. However, you could set a goal to proactively contract portions of your inputs (e.g. feed) and outputs (e.g. piglets, market hogs) or use risk management strategies to protect farm profits that will allow you to endure low market conditions.

Additional strategies to help manage your stress can include accepting your imperfections and forgiving yourself for hard days when you struggle with your emotions. When overwhelmed with feelings of stress, try not to lose sight of what is most important and respond to others with kindness – both co-workers and family.

Be kind to yourself. Certain times and situations may have been difficult; however, each day of your life is a new blank page to fill. The practice of positive thinking can improve your physical and mental health, and new perspectives and optimism can bring renewed strength to managing stress. Reflect on how daily animal care tasks are going on the farm and discuss ways to minimize stress or create efficiencies. How will you begin to fill those blank pages with positive thoughts?
