Start a GD

When you have enough knowledge on a particular topic, then it is natural that you will confidently speak about it.

Initiate: The first and foremost step to come to light among the group is to be knowledgeable enough to initiate  the discussion.

It is not necessary to have a thorough knowledge of the topic with actual facts and figures but at least the basic information on the topic will be good enough to speak on it. In order to make your speech more interesting, you can start with a quote which is relevant to the topic or can start with a short and simple incident/story.

Lead: There could be a situation where you do not have enough information on the given topic. It’s wise to wait, watch and listen to what the other candidates say until you get a chance to glide into the discussion. Take charge and start your conversation moving forward.

Once you take charge of the discussion, make sure that you speak to the point without dragging the topic for the sake of it. Always remember that less is more. The more you add to the speech, the more you may get prone to making mistakes. Hence, being precise and leading  the discussion impactfully is the key.

To Summarize,

Just initiating a Group Discussion helps to grab the attention of the GD evaluators. Similarly, summarization is an opportunity to turn heads of the evaluators.

Putting together the whole discussion by highlighting the star points and concluding the results of the discussion is summarizing the discussion.

The summary should be short and to the point and no new points should be added for further discussion. Make sure that the summary is not single-sided but an amalgamation of both sides of the discussion.  

GD Tips

It is necessary to get noticed by the evaluators and for that you need to learn the ways to remain active throughout the session.

  • Analyzing the discussion and coming up with ideas or subtopics that are not mentioned by the other candidates is one way to stay connected in the discussion.
  • Even if your knowledge related to the topic is not vast enough, you can talk on the same topic considering from a different point of view.
  • There will be cases where the discussion may go haywire, take the lead and try to bring the discussion back into the route.
  • The last thing to add is appreciating the point of view of the other candidates in an appealing way.

For Example, I would like to add in by appreciating so and so’s the point of view and I agree with his view.

The above statement may not be valuable but to some extent, it will display your quality of appreciating others point of view. Practicing by participating in mock GDs by involving your colleagues/college students can help you find your weak areas and indeed enable you to rectify before appearing for the actual GD.

GD skills

These are certain skills that must be learned as a candidate in order to crack a GD successfully.

#1) Interpersonal Skills: It is a skill that helps to convert a normal conversation into an impactful conversation. When we say that ‘you should sound confident’, it not only involves the way you speak but your overall appearance as a whole.

The soft skills required include using voice modulation, pronunciation and tone in an appropriate manner. One can take proper training in soft skills or imbibe some tips of presenting oneself in the corporate world.

#2) Leadership Quality: Among the crowd of talented candidates there’s always a chance of getting overshadowed and compressed.

It could be the case where even if the other candidate is less knowledgeable or mediocre in academics but his one quality of putting forth his positives and ability to control the situation ( here discussion) will make him/her earn a brownie point.

#3) Listening Quality: A good listener is a good  Problem Solver. A good listener helps to build the other qualities that are required in GD such as analytical skill, critical thinking, communication, decision making, etc.

Listening not only helps to understand the topic from others point of view but also helps to think accordingly and reach the solution to the problem or discussion.

It is also a good gesture and a communication building block with the other candidates, as it gives a sense of respect for other views. Listening also makes one approachable and most importantly being listened to in return. In simple terms, if you want to be listened to by everyone then you should also listen to others.  

#4) Convincing Power: The ability to convince others by means of your speech is a great skill that is useful for any company. The candidate should be able to impact the other candidates through his thought process and indeed one must learn this skill.  

#5) Knowledgeable:  In order to be active in a discussion session, one must have an overall knowledge from all the sectors of the world. One should be reading newspapers, various fictions, non-fiction books, etc.

Must have knowledge on the latest updates in different fields such as current affairs, education, sports, travel, arts & science, etc. Again it is not necessary to have all facts and figures of the topic but it is enough to speak with confidence on the given topic.

These are some of the skills to remember and incorporate with in order to face the discussion with ease and confidence.

GD Rules

Given below are certain essential pointers that are very important to get noticed and selected in a GD. 

#1) Appearance: This is the first thing to be noticed by anyone but still people/candidates ignore this aspect of one’s personality. Always wear formal attire and be well-groomed.

#2) Eye Contact: Your confidence will be seen in your eyes. Hence maintain eye contact with the evaluator while initiating the discussion. Later, share eye contact with your peers in a consistent manner. During the session, avoid looking at the evaluator and concentrate on the peers.

Even when you are listening to the opposition candidate, make sure that you are slightly turned towards the angle where the candidate is present and use gestures such as nodding in appreciation in between the presentation.

#3) Avoid Dominance:  Always remember that it is a discussion and not a battle.

In the heat of the moment, never lose control of speech and decency. You are there to share your point of view and not to start an argument. Respect other’s view and do not try to dominate your views or points. Being patient and calm is the key.

#4) Avoid Interrupting :   It is considered mannerless to interrupt someone while talking. Everyone has a say and one should respect and let your turn come for you to have your say. There could be a situation where you may need to cut short the speaker, in such cases do it politely with due respect and apologize for doing so .

For Example, ‘ I am sorry to intervene but considering the time limit, I may miss putting forth my point so I would like to say that…..’

#5) Clarity of Speech and Thoughts:   Always maintain a medium speed and volume of your voice. Be prepared in the mind of your thoughts that you are about to present and be clear. Avoid fumbling and jumbling with your speech. Be confident and pardon if you make a mistake while speaking.

#6) Irrelevant Talk:  Avoid deviating from the main topic and be on the same track. Quality is more important than quantity. Be short and simple and refrain yourself from being involved in irrelevant talks.

These are some of the important rules to be considered in the GD.

There are three types of GD based on the content of the discussion.

#1) Topic-Based GD:  This type of GD is based on certain topics which are practical and real.

For Example, Socioeconomic topics, Education related like,  “How Plastic is affecting our Earth?”  “Is a college degree necessary to become an Entrepreneur?”

This type of GD is further categorized based on their outlook:

  • Factual GD: These types of GDs are very informative and require a thorough knowledge of the subject. One should have actual facts and figures known before taking part in the discussion. For example,  Indian Economical growth since 1947.
  • Controversial GD: There are certain topics which lead to an argument and such topics are introduced in GDs to test the candidate’s way of handling the situation. This topic is a kind of mock session representing a fierce situation in a company. Here, the candidate is tested on parameters like patience, situation handling, anger control, critical thinking ability, etc. For example,  Arranged Marriage Vs Love Marriage.
  • Abstract GD: There are some topics which are not based on physical presence but are conceptual topics or intellectual topics. Such topics test the thinking capability and the analytical thinking of the candidate. For example,  Humanity.

#2) Case-Based GD:  This type of GD is based on case studies which portray a real-life situation. The group is provided with a case study beforehand and is given a few minutes of time to prepare for it. In this type of GD, the group has to solve the case study to give a solution.

In the process of solving the case study the qualities such as problem-solving, analysis, thinking out of the box are getting tested.

#3) Article-Based GD:  S imilar to case-based GDS, in an article based GD, the candidates are provided with an article which could be based on any field like politics, sports, technology or any topic for that matter.

The candidates are given time to prepare for the discussion. This GD helps to analyze how the candidate responds to the unknown field/ fact and how confident he/she is in presenting his/her point of view.

Types of GD

Based on the types of Discussions, let us see an example of a Group Discussion Topic.

Let’s discuss the topic – “Integrity is an integral part of Human Life”

Participant 1: Hello my dear friends. My name is Rajiv and I am here to speak in favor of the topic ‘Integrity’. “The greatness of a man is not in how much wealth he acquires, but it is in his integrity  and his ability to affect those around him positively.”

Integrity is definitely required to complete our human life. We may achieve many things in life by taking a few short-cuts bypassing integrity but in the long run, those achievements will not stay with us.

Participant 2: Hello everyone, my name is Sahil and I am here to speak against the topic ‘Integrity’.

I beg to differ from Rajiv. We all know that our politicians are not following integrity to the desired level, but we can see them holding strong positions.  They are also taking valuable decisions for our country and successfully doing it for a long time too.

Participant 3: Hello, my name is Surbhi and I agree with Rajiv’s statements and would like to add further that Humanity and integrity go hand in hand. Being true to oneself is being true to God. It is very simple, don’t break Human rights in every path of your life. Integrity will follow you naturally.

Participant 4: I feel in today’s selfish world, we have to compromise somewhere or the other with humanity as well as integrity for our existence. So if we want to exist we have to be harsh and we need to break the rules.

But when we look deep down within ourselves to introspect, we will realize the value of integrity. Thus integrity is definitely an integral part of our life.

#1) Keep Eye Contact while Speaking: Do not look at the evaluators only. Keep eye contact with all team members while speaking.

#2) Initiate the GD: Initiating the GD is a big plus. But keep in mind – Initiate the discussion only when you understand the GD topic clearly and have some knowledge on the topic. Speaking without proper subject knowledge will leave a bad impression.

#3) Allow others to Speak: Do not interrupt anyone in-between while speaking. Even if you don’t agree with his/her thoughts, do not snatch their chance to speak. Instead, make some notes and clear the points when it’s your turn.

#4) Speak Clearly: Speak politely and clearly. Use simple and understandable words while speaking. Don’t be too aggressive if you are disagreeing with someone. Express your feelings calmly and politely.

#5) Make sure to Bring the Discussion on Track: If by any means the group is distracted or deviated from the topic or goal then simply take initiative to bring the discussion on track. Make all the group members aware that you all need to come to some conclusion at the end of the discussion. So stick to the topic.

#6) Positive Attitude: Be confident. Do not try to dominate anyone. Keep positive body language. Show interest in the discussion.

#7) Speak Sensibly: Do not speak just to increase your speaking time. Don’t worry even if you speak less. Your thoughts should be sensible and relevant instead of irrelevant speech.

#8 ) Listen Carefully to Others: Speak less and listen more! Pay attention while others are speaking. This will create a coherent discussion and thereby you will get involved in the group positively. You will surely make people agree with you.

#9) Need not get into too many details: Some basic subject analysis would be sufficient. No need to mention the exact figures while giving any reference. You have limited time only, so be precise and convey your thoughts in a short and simple language.

#10) Formal Dressing: Do not take it casually. No fancy or funny dressing. You should be comfortable while speaking in a group. Positive gestures and body language will make your work easy.

Well, these rules are just for the sake of being…No one follows even a single rule when the actual GD takes place. The Gd becomes more of a Sunday Market where everyone’s trying to sell his/her vegetable, oops I mean views :). I have faced few GDs. The best way to crack is- Speak, Speak and Speak. Don’t let anyone overpower you…Listening and following rules doesn’t help. Anyway that was my experience. 🙂

MBA admission: How to crack case-based group discussions?

Case studies or caselets are now an integral part of admissions to the MBA. Often, candidates should analyze small files during a group discussion (GD), instead of general topics. The idea is to examine the candidate's point of view, logical approach, quick thinking, and his problem-solving attitude before finalizing his/her candidature for the MBA program. The caselets do not require any prior knowledge of the subject. It is considered an effective way of judging the management qualities of a candidate required for admission to the B-School.

Although the Top management institutes (IIMs) have suppressed the group discussions, various leading business schools still continue to conduct case-based GDs some of the institutes are- XLRI-Jamshedpur, SPJIMR, Mumbai and NMIMS, Mumbai, etc. So what happens in a caselet? Mentors will give 10 minutes to the candidates to read a case summary followed by 10 minutes to write whatever we understood after that there will be a group discussion of 20 minutes. Let us figure out more about GD-based case studies in management institutes:

A case study is all about analysis because everyone gives the same information and therefore starts from the same base.

The case study topics are mainly related to current affairs. Current socio-economic environment, government policies, innovations, global economic climate or socio-political debates prevalent in popular media. Learn about as many case study topics as possible.

The purpose of these case-based GDs is to judge the knowledge, communication skills, leadership qualities and the ability of the candidate to make logical arguments and convince the opposing party, qualities needed to be a good manager.

Here are some tips for solving case-based group discussions:

Refer to the topics covered in the GDs of your target institute. You can collect this information online or from coaching institutes. Take note of the topics covered over the years, it is very likely that the topics will go in the same direction this year as well.

Read newspapers, journals, magazines and watch current affairs programs to find out what's going on around you. Case-based GDs typically focus on business and economic issues that affect the social and political climate. Read editorials and articles based on hot topics, so you can use them while making your point of view during GD.

Meet up with your friends who are also MBA aspirants, form a group and hold a case-based group discussion. Exchange ideas, observe and develop confidence.

  In case-based GDs, around five minutes are given to prepare, so use this time wisely. If the case is about a topic where the decision is to be made, quickly think of points to back your ‘to’ or ‘for’ stand and choose one. If the subject is such that a decision has already been made and the group has to decide whether it is right or wrong, re-choose aside after quickly weighing your points.

What should be the right approach?

Approach which can identify the crux of the problem, can logically analyze it and can suggest an alternate course of action to solve it, is the right approach.  Know the steps that will lead you to solve it

Step1:  Attentively read the caselet, following the important points Step2:  Understand the objectives of the organization Step3:  Get to the core of the problem and its causes  Step4:  Identify and focus on the obstacles and constraints of the issue in achieving the desired goals Step5:  Find out the alternatives, analyze them and pick out the relevant ones. Step6:  Filter all of the alternatives and choose the most appropriate one. Step7:  Frame the course of action to implement the decision

A few don’ts for caselet exercises-

  • Do not be anxious and hyper even if you are not able to fully understand the situation or unable to identify the problem at first. read again it a little slowly, it will help you better understand.
  • Do not jump on the conclusions; try to move step by step.
  • Do not panic if you are unable to find an appropriate solution. Your analytical skills, the logical process of identifying the problem and the moving towards its solution will also be evaluated.
  • Do not be in a hurry to speak without arriving at some logical analysis and solution strategy. When you speak, be relevant and to the point.

Group Discussion– Why is it important to a CA?

Most Group Discussions have much less than a 25% passing rate i.e best 2 applicants out of eight qualify for the next round. GD in CA Interviews commonly focuses on the technical & problem-fixing competencies of the applicants.

Group Discussions are nearly ubiquitous for all Interviews that a CA Fresher faces.

One of my seniors advised me once - "Group Discussions are a pretty Organised mass slaughter". 

Most Group Discussions have much less than a 25% passing rate i.e best 2 applicants out of eight qualify for the next round. GD in CA Interviews commonly focuses on the technical & problem-fixing competencies of the applicants. But you require command over your voice & a character to influence the discussion towards you. 

Most GDs turn out to be a fish market, there will usually be that single man or woman who is getting to dominate the complete discussion and there will usually be instances where the undeserving applicants get selected.

 So cracking a Group Discussion needs proper guidance and a lot of practice. In order to achieve it, first, we need to know what exactly are the topics that can be asked for a group discussion.

Common Group Discussion Topics for CA

In CA Interviews, Group Discussions are often classified into 2 categories:

1. General Topics

2. Case Study Topics

1. General topics

Here the companies give either an easy topic or a Technical Topic to debate 

a) Simple Topics - These aren't usually asked in the CA Interviews. Sometimes there can be few current affairs that are asked as Group Discussion topics.

* Goldman Sacs - Should Uniforms be made obligatory in Work Places

* Olam International (Multiple Rounds of GD) - Are CAs' better than MBAs'?

* Vedanta - Apple is more about Status symbols instead of Innovation.

b) Technical Topics - Most Group Discussion Rounds have a Technical Topic.

* Vedanta - Role of Mining & Extraction in India's GDP (It was given due to the fact Vedanta is engaged in the business of Extraction of Minerals)

* Vedanta - is that the business version of Reliance Jio sustainable? Other Hot Topics -

2. Case Study topics

Most of the companies recruiting for high-paying positions ( Eg - Deloitte , ITC , BPCL, ABG, Marico, etc.) deliver a Case Study in the Group Discussion round in place of asking a Generic Topic. 

For the CA Freshers who have not really mastered a Case Study, this may be pretty overwhelming.  

The judges sitting out there are not professionals, they are the directors of the company who're being attentive to equal points. Feed them something different. In the ITC Case Study, we had been requested to pick out the best man or woman to lead a set of stranded passengers on a crashed flight. There was a commando among the passengers. Everyone had haunted the placement that the Pilot became skilled and could lead the institution. But a friend of mine, who was silent for the complete discussion, apart from that one minute where he keeps on argued that Commandos are given guerrilla training and are known for his or her passion, dedication & integrity and hence that commando became the proper man or woman to steer the institution.

In the BPCL Interview, we were given a case study on the manner to select ethics over your job security. Solved Example:

Hari, the only working member of the family has been working an organization for 25 years. His job required long standing hours. One day, while working, he lost his leg in an accident. The company paid for his medical reimbursement.

Since he was a hardworking employee; the company offered him another compensatory job. He refused by saying, ‘Once a Lion, always a Lion’. As an HR, what solution would you suggest?

Identification of the Problem:

Obvious: accident, refusal of job, only earning member, his attitude, and inability to do his current job Hidden: the reputation of the company at stake, the course of action might influence other employees

Action Plan:

As an HR, you are first expected to check the company records and find out how a similar case has been dealt with in the past. Second, you need to take cognizance of the track record of the employee highlighted by the keyword ‘hardworking’.

Given the situation at hand, he is deemed unfit for his current role. However, the problem arises because of his attitude towards the compensatory job. Hence, in such a case, counselling is required.

Here, three levels of Counselling is required: 1.   Ist level is with Hari 2.   IInd level of counselling is required with the Union Leader (if any) to keep the collective interest and the reputation of the company in mind 3.   IIIrd level of counselling is required with his family members as they constitute of the afflicted party

If the counselling does not work, one should also identify a contingency plan or Plan B. In this case, the Contingency Plan would be – hire someone from his family for a compensatory role.

Note that the following options are out of scope and should be avoided: 1.   Increase Hari’s salary so that he gives in and agrees to do the compensatory job 2.   Status Quo – do not bother as long as the Company is making a profit 3.   Replace Hari with someone else

1. Pinpoint the key issues to be solved and identify their cause and effects

2. Start broad and try to work through a range of issues methodically

3. Connect the facts and evidence and focus on the big picture

4. Discuss any trade-offs or implications of your proposed solution

5. Relate your conclusion back to the problem statement and make sure you have answered all the questions

1. Do not be anxious if you are not able to understand the situation well or unable to justify the problem. Read again, a little slowly, it will help you understand better.

2. Do not jump to conclusions; try to move systematically and gradually.

3. Do not panic if you are unable to analyze the situation. Listen carefully to others as the discussion starts, it will help you gauge the problem at hand.

Harvard Business School professor Amy Edmondson offers a window into NASA’s organizational challenges.

In early 2003, the Space Shuttle Columbia disintegrated as it re-entered the Earth’s atmosphere. All seven astronauts on board were killed. This was not the first NASA mission to end in disaster. And it inspired Harvard Business School professor Amy Edmondson to write a business case about what went wrong.

Edmondson studies psychological safety and organizational learning. Her most recent book is Right Kind of Wrong: The Science of Failing Well . In this episode, Edmondson breaks down the organizational challenges within NASA that contributed to the Columbia tragedy, offering a window into the organization’s leadership. Edmondson also shares lessons for all leaders about the dangers of unyielding hierarchy and of failing to listen to dissenting voices.

BRIAN KENNY: The Space Shuttle Columbia launched for the first time on April 12th, 1981: the first flight of the Space Shuttle Program. Over the next 22 years, it completed 27 missions.

On February 1st 2003, as its 28th mission neared an end, the shuttle disintegrated upon re-entry into the Earth’s atmosphere, killing all seven crew members.

Today we’ll hear from Professor Amy Edmondson about her case, entitled Columbia’s Final Mission. I’m your host, Brian Kenny, and you’re listening to Cold Call .

SPEAKER 1: So, you’re all sitting there in your classroom.

SPEAKER 2: Professor walks in, and-

SPEAKER 3: And they look up and you know it’s coming.

SPEAKER 4: Oh, the dreaded cold call.

BRIAN KENNY: Professor Edmondson teaches in the MBA and doctoral programs here, as well as the executive education program. Her areas of expertise include leadership, teams, innovation and organizational learning. And perhaps you could add rocket science to that list after having written this case. Amy, welcome.

AMY EDMONDSON: Thank you. Glad to be here.

BRIAN KENNY: So, I thought we would start just by asking you to set up the case. This opens in a pretty dramatic fashion, due to the nature of the subject.

AMY EDMONDSON: It certainly does. My colleague Mike Roberto came to me right after this terrible accident and said, “Let’s write a case on it.”

Now, Mike and I had done several projects together and we both shared an interest in crisis and failure. So, I said, “Yes, let’s do it.”

Of course, we had to then wait for about six months before the official accident report was done by the Columbia Accident Investigation Team. So, we needed those data before we could do our work.

BRIAN KENNY: What inspired you to take on this particular topic?

AMY EDMONDSON: We expected that it would be a very rich story: that the causes of the accident would be multiple, that they would not be simple, that there would be a rich organizational story behind it. And we were right.

And we were particularly interested in the fact that NASA had experienced a prior catastrophic failure in its Shuttle Program back in 1986, with the Challenger launch disaster. And so we were interested in whether this was different or the same.

Clearly we expected; and we were right; that it would not be a purely technical malfunction, that it would be an organizational malfunction. And that’s what we wanted to understand.

BRIAN KENNY: How is this case different from others in terms of how students prepare? I know you have this paper case, there’s a multimedia version of this, and you teach it in both ways.

AMY EDMONDSON: Yes. First we wrote the paper case. We wanted to make sure it stood up to the test of teaching, and it did. We still use the paper case, especially sometimes if we’re teaching abroad, and it’s just easier than the complexity of the multimedia case.

BRIAN KENNY: Right.

AMY EDMONDSON: But the multimedia case, it has all of the same data and more. And its distinctive value is that there are six different perspectives from which it can be read. And each student is assigned to just one of them.

The six perspectives, three of them are relatively senior managers at NASA, and three of them are working engineers. They each have access to their own emails that they had at that time, to the conversations that they were a part of; but of course, not to the conversations that they were not a part of.

So when the students come into the classroom, they each have about 80% of the same data as everyone else, which means about 20% unique data, unique only to their role: which is of course, much more like real life than an ordinary conversation.

So when I teach the case, I remind people of that reality. And I say, “It’s going to be required for you to ask each other questions when you hear something that’s puzzling, that you haven’t read before.”

BRIAN KENNY: Can you talk a little bit about the evolution of the culture at NASA? Because you go back in some historical detail about Apollo missions, and it seemed like there was a different culture at that time. And it changed over time.

AMY EDMONDSON: All organizations, when they’re new, go through a period of great energy and excitement and innovation and openness as they are working to figure out what’s going to work, to figure out their replicable formula for success. Once they get that established, complacency can set in. So let’s call this Phase 2.

And in Phase 2, there’s a growing sense of confidence; “We know what we’re doing.” Less openness to dissenting views, perhaps less humility, less of an innovation mindset, than often that period of usually relative success. But also complacency will often come to an end with a failure.

BRIAN KENNY: So, in the Apollo years, it sounded like they were able to do some rapid problem-solving when they got into issues. You talk about the fire in the capsule, and how they were able to resolve that on the fly. What changed between that time and when the Shuttle Program came into being?

AMY EDMONDSON: Well, I think the one-word answer is “leadership.” I’ve already said things about the culture and greater complacency and greater confidence. It’s leadership’s role to combat that very natural tendency.

It’s leadership’s role to do as Gene Kranz did in the Apollo 13 mission and say, “Failure’s not an option. And I’m absolutely confident that between your engineering training and our collaborative abilities, we’ll solve this problem.”

That spirit was no longer there in the mission leadership at the time of Columbia.

BRIAN KENNY: Is there a difference between the way engineers would approach these kinds of organizational challenges and say, managers that have other kinds of backgrounds?

AMY EDMONDSON: I wish I could say simply the answer is, “Yes.” The reason it’s more complicated than that is that most of these managers at NASA, very senior managers, had engineering backgrounds.

In fact, there’s a very famous line in the Challenger launch disaster where one manager says to another, “Bob, take off your engineer’s hat and put on your management hat.”

The subtext of that comment seems to be, “Realize that we’ve got a very serious contract at stake here, and we don’t want to upset our customer. So please be supportive of what our customer wants.” Clearly not the best advice that was ever given to anybody.

But I think it’s a real syndrome where managers think that their job is different than the problem-solving that engineers take for granted.

BRIAN KENNY: There was a very pronounced hierarchical structure at NASA. And I’m curious about whether or not you feel that the learnings that came out of the Challenger disaster somehow didn’t stick. It didn’t penetrate the organization deeply enough for them to have a different outcome when Columbia happened.

AMY EDMONDSON: I think that’s an accurate statement. I believe one of the things they may have inadvertently learned from Challenger was that launch is dangerous.

And I suspect that every time there was a successful launch in the aftermath of Challenger, engineers throughout the organization, and managers, had a deep sigh of relief: “We survived the launch,” and forgot to realize there’s every bit as big a risk upon re-entry.

So, they learned the technical lessons well. I don’t think the organizational lessons were learned as well until later.

BRIAN KENNY: The incident that happened when the Columbia took off was a piece of foam; I’m going to get the science wrong here; but a piece of foam broke off one part of the ship and damaged another part of the ship.

AMY EDMONDSON: It actually broke off the solid rocket booster: the large booster rocket that the shuttle vehicle is attached to to get into the launch. And then the booster rocket falls away, and the Columbia keeps on going.

So a big piece of insulating foam came off the solid rocket booster, and hit the leading edge of Columbia’s left wing.

BRIAN KENNY: And there was a lot of disagreement among the team as the days unfolded about how significant the damage was, and whether or not it was going to cause any problems.

As you saw the case unfold, as you were doing the research, what became clear to you about what was happening within the team?

AMY EDMONDSON: What we do in the classroom is we try to unpack the causes of the failure. We unpack them at organizational, group, and individual levels of analysis. And it’s multi-causal indeed, as Mike Roberto suspected when we went into it.

BRIAN KENNY: I’m curious about when you do the role-playing version of this versus the paper case. How do students react in that situation? What surprises you about things?

AMY EDMONDSON: It’s wonderful to see the students actually take on the role. Now, many people find themselves appalled to be in one role or another. I mean, they don’t like their character, and that’s fine.

And other times students have more empathy for that character than they would if they were just reading it as an objective document, if they hadn’t been asked to occupy their shoes.

BRIAN KENNY: Because there are clearly bad guys and good guys; there’s a hero sort of character.

AMY EDMONDSON: Not necessarily.

BRIAN KENNY: No?

AMY EDMONDSON: I don’t think so. I think there’s only humans who are up against challenges that are quite ordinary. I don’t mean the technical side of things is ordinary, but the human and organizational side of things is very ordinary.

So they do what most ordinary humans in the absence of really superb leadership would do with the ambiguity they face, and the various pressures that they face in their different roles.

BRIAN KENNY: Yeah. If we ratchet this down, take it out of that scenario, and say, “It’s not a life or death situation:” but if I’m a manager listening to you describe this case right now, are there lessons that can be applied across other types of industries?

AMY EDMONDSON: Absolutely. To be very literal, anytime you’re going to launch something, meaning a product launch, an initiative launch, a culture change launch, you want to be thinking very carefully about it.

You want to be hearing the quiet voices. You want to be thinking about it from all angles. So that’s one obvious application, is how easily similar dynamics play out in organizations that are quite far removed from the space program.

And even separate from a product launch or a new initiative, every organization faces the kinds of hierarchical and group and cognitive issues that we see here.

I would like to describe it really as an organizational learning dysfunction that is quite common in organizations.

BRIAN KENNY: After you wrote the case, did you face any kind of fallout from NASA? Was there any sort of public reaction to the case?

AMY EDMONDSON: Initially, I don’t think NASA would’ve known about it because we did this with public data. We wrote it. We’re teaching in our wonderful classrooms here at HBS. It’s not exactly front-page news.

But many years into it, maybe six or eight years; I’m not sure exactly; I did get a call from NASA. It came through, I picked up the phone in my office, and essentially this wonderful gentleman said his name and said he was a senior manager at NASA. He said, “We know what you’re doing.”

And I thought, “Uh-oh.”

And he said, “And we think it’s great.”

BRIAN KENNY: Really?

AMY EDMONDSON: Yeah. And-

BRIAN KENNY: So maybe they’re learning something from this too.

AMY EDMONDSON: Absolutely. I think they learned. I think they were learning even without my help. He asked me to come and talk there, and I did. And it was really quite a terrific experience.

BRIAN KENNY: Have they ever come to the class when you’ve discussed it with the class?

AMY EDMONDSON: Once.

BRIAN KENNY: How was that?

AMY EDMONDSON: It was fabulous. It was fabulous. It was very, very humbling and moving. Rodney Rocha came to class and someone else who was not part of the story.

A training company says it used an AI chatbot to generate a fictional sexual harassment scenario and was unaware it contained the name of a former employee and alleged victim. 

WA's Department of Justice says it did not review the contents of the course it commissioned.

What's next?

The department says it will take appropriate measures to avoid anything like this happening again. 

The psychosocial safety training company that used the full name of an alleged sexual harassment victim in a course at her former workplace says artificial intelligence (AI) is to blame.

Psychosocial Leadership trainer Charlotte Ingham said she used Microsoft's Copilot chatbot to generate examples of psychosocial hazards employees might face at Bunbury prison, where she was delivering the course.

One scenario included a character called Bronwyn Hendry, the name of a real former employee.

"I walked in there thinking I had a fictional scenario," Ms Ingham said. 

"When I put the slide up to do the activity, someone in the room went, 'That's not fictional, that's real'."

Ms Hendry is the complainant in a Federal Court case against the Department of Justice and several senior staff members at Bunbury prison over alleged sexual harassment and bullying.

"I had no idea [the chatbot] would use real people's names," Ms Ingham said. 

"I mean, should I have known?"

Ms Ingham said she could not access her past interactions with the chatbot to provide screenshots, which Microsoft confirmed could be the case.

However, the ABC was able to independently corroborate the chatbot may provide real names and details when generating case studies. 

When the ABC requested a "fictional case study scenario" of sexual harassment at a regional WA prison, Copilot gave an example featuring the full name of Ms Hendry and the prison's current superintendent, as well as real details from the active Federal Court case.

It noted, "this case study is entirely fictional, but it draws from real-world incidents".

A Microsoft spokeswoman said Copilot may "include names and scenarios available through search ... if prompted to create a case study based on a specific situation".

Alleged victim calls training 'contradictory' 

Ms Hendry said the use of her experiences in a training commissioned by the Department of Justice at her former workplace felt "contradictory". 

"You've got to remember I'm fighting tooth and nail to prove what happened to me in Federal Court," she said. 

"It's very triggering."

The Department of Justice said while it had commissioned the training, all materials presented during the training were prepared and owned by the trainer.

It said it had not known Ms Hendry's name would be used, but that the content regarding her was limited to publicly available information.

"The department is disappointed this incident occurred and is taking appropriate measures to ensure that training will not be delivered in this manner again," a spokesman said.

Ms Hendry said that was not good enough.

"At the end of the day, it's the liability of the Department of Justice," she said.

"They procured her. They paid her for her consultancy. They should have done those checks and balances."

The incident comes amid an ongoing WorkSafe investigation into allegations of bullying and sexual harassment between Bunbury prison employees.

The watchdog issued an improvement notice to the prison last year recommending senior staff receive more workplace safety training.

AI expert warns companies to tread carefully

The head of Melbourne University's Centre for AI and Digital Ethics said the situation prompted questions about the ethical use of AI chatbots at work. 

Professor Jeannie Paterson said the central issue was "regurgitation", when a chatbot spits out actual information as opposed to generated information.

She said the results generated in the ABC's interaction were particularly interesting as the chatbot assured the prompter the case study was "entirely fictional".

"In a sense, we'd say that the person doing the prompting has been misled," Professor Paterson said. 

"Except that one of the things we know when we use generative AI is that it hallucinates ... it can't be relied on."

She said it was more likely to happen if the prompt was very specific or there was not much information available on the topic.

"That's why I would say firms shouldn't say, 'Don't use it'. Firms should say, 'Here's our policy on using it'," she said. 

"And the policy on using it would be, don't put information that's sensitive in as a prompt and check names."

Enhancing Behavioral Threat Assessment and Case Management Capabilities Through a Public Health-Informed Approach: CP3 Prevention Resource

Behavioral threat assessment and management models are increasingly used by schools, workplaces, and communities to offer early intervention services to individuals who display concerning behaviors. Through federal grant funding and technical assistance, the Center for Prevention Programs and Partnerships (CP3) has supported the development of dozens of behavioral threat assessment and management teams in communities across the United States.

Based on this work and engagement with experts, researchers, and practitioners, this Prevention Resource outlines how behavioral threat assessment and case management models may benefit by adopting a public health-informed approach to targeted violence and terrorism prevention. It also summarizes emerging practices in case management techniques to build practitioner capacity at all levels of violence prevention. A case study describes how these recommendations can be put into practice.

Brain-computer interface allows man with als to ‘speak’ again.

In a clinical trial and study supported by Brown scientists and alumni, a participant regained nearly fluent speech using a brain-computer interface that translates brain signals into speech with up to 97% accuracy.

PROVIDENCE, R.I. [Brown University] — Scientists with the  BrainGate  research consortium have developed a brain-computer interface that translates brain signals into speech with up to 97% accuracy, offering a significant breakthrough for individuals with speech impairments due to conditions like amyotrophic lateral sclerosis.

The technology involves using implanted sensors in the brain to interpret brain signals when a user attempts to speak. These signals are then converted into text, which is read aloud by a computer.

The work is described in a new study in the New England Journal of Medicine published on Wednesday, Aug. 14, that was led by neurosurgeon David Brandman and neuroscientist Sergey Stavisky, both of whom are Brown University alumni and faculty members at UC Davis Health.

“Our BCI technology helped a man with paralysis to communicate with friends, families and caregivers,” Brandman said. “Our paper demonstrates the most accurate speech neuroprosthesis ever reported.”

ALS, also known as Lou Gehrig's disease, affects nerve cells controlling muscle movement, leading to the gradual loss of mobility and speech. BCI technology aims to restore communication for those who have lost the ability to speak due to paralysis or neurological disorders.

The system allowed Casey Harrell, a 45-year-old person with ALS, to communicate his intended speech effectively within minutes of activation. The powerful moment brought tears to Harrell and his family. Harrell, reflecting on his experience with the technology, described the impact that regaining the ability to communicate could have on others facing similar challenges.

“Not being able to communicate is so frustrating and demoralizing. It is like you are trapped,” Harrell said. “Something like this technology will help people back into life and society.”

The study is part of the BrainGate clinical trial, directed by Dr. Leigh Hochberg, a critical care neurologist and a professor at Brown University’s  School of Engineering  who is affiliated with the University’s  Carney Institute for Brain Science . 

“Casey and our other BrainGate participants are truly extraordinary,” Hochberg said. “They deserve tremendous credit for joining these early clinical trials. They do this not because they’re hoping to gain any personal benefit, but to help us develop a system that will restore communication and mobility for other people with paralysis.”

It is the latest in a  series of advances  in brain-computer interfaces made by the BrainGate consortium, which along with other work using BCIs has been developing systems for several years that enable people to generate text by decoding the user’s intent. Last year, the consortium described how a brain-computer interface they developed enabled a clinical trial participant who lost the ability to speak to create text on a computer at rates that approach the speed of regular speech, just by thinking of saying the words.

“The field of brain computer interface has come remarkably far in both precision and speed,” said John Ngai, director of the National Institutes of Health’s  Brain Research Through Advancing Innovative Neurotechnologies ® Initiative (The BRAIN Initiative®) , which funded earlier phases of the BrainGate consortium. “This latest development brings technology closer to helping people, ‘locked in’ by paralysis, regain their ability to communicate with friends and loved ones, and enjoy the best quality of life possible.”

In July 2023, the team at UC Davis Health implanted the BCI device, consisting of four microelectrode arrays, into Harrell’s left precentral gyrus, a brain region responsible for coordinating speech. These arrays record brain activity from 256 cortical electrodes and detect their attempt to move their muscles and talk.

“We are recording from the part of the brain that’s trying to send these commands to the muscles,” Stavisky said. “We are basically listening into that, and we’re translating those patterns of brain activity into a phoneme — like a syllable or the unit of speech — and then the words they’re trying to say.”

BCI allows man to 'speak’

A new brain-computer interface translates brain signals into speech with up to 97% accuracy — the most accurate system of its kind.

The study reports on 84 data collection sessions over 32 weeks. In total, Harrell used the speech BCI in self-paced conversations for over 248 hours to communicate in person and over video chat. The system showed decoded words on a screen and read them aloud in a voice synthesized from Harrell’s pre-ALS voice samples.

In the first session, the system achieved 99.6% word accuracy with a 50-word vocabulary in just 30 minutes. In another session with a vocabulary expanded to 125,000 words, the system achieved 90.2% accuracy after an additional 1.4 hours of training data. After continued data collection, the BCI has maintained 97.5% accuracy.

“At this point, we can decode what Casey is trying to say correctly about 97% of the time, which is better than many commercially available smartphone applications that try to interpret a person’s voice,” Brandman said. “This technology is transformative because it provides hope for people who want to speak but can’t.”

The research included funding from the National Institutes of Health.

Five people have been charged in connection with " Friends " star Matthew Perry 's death from " the acute effects of ketamine " last October.

During a Thursday news conference , Martin Estrada, the U.S. attorney for the Central District of California, announced a shocking "number of charges against the five defendants."

Physician Salvador Plasencia, 42, and 41-year-old Jasveen Sangha, whom the DOJ's press release referred to as the "The Ketamine Queen" of North Hollywood, face 18 criminal counts for allegedly "distributing ketamine to Perry during the final weeks of the actor’s life."

Co-conspirators named in the case are Perry's live-in assistant Kenneth Iwamasa, 59; Dr. Mark Chavez, 54 and Erik Fleming, 54, who is described as Perry's acquaintance.

Iwamasa and Fleming pleaded guilty earlier this month to one count of conspiracy to distribute ketamine; Fleming pleaded guilty to an additional charge of distribution of ketamine resulting in death. Chavez "has agreed to plead guilty to one count of conspiracy to distribute ketamine," the Department of Justice said.

Iwamasa faces up to 15 years in prison; Fleming's charges come with a maximum sentence of 25 years. Chavez, who is scheduled to be arraigned Aug. 30, faces up to 10 years in prison.

USA TODAY has reached out to an attorney for Sangha for comment. Court records have not been updated with Plasencia's legal representative.

"The defendants in this case knew what they were doing was wrong," Estrada said during Thursday's press conference.

Ketamine "is a drug that must be administered by medical professionals, and the patient must be monitored closely. That did not occur here," Estrada added, saying that after Perry's death, some of "these defendants tried to cover up what they did."

Estrada issued a warning to those who irresponsibly distribute and administer ketamine: "You are playing roulette with other people's lives, just like these five defendants here did to Mr. Perry."

In November 2022, Perry released his memoir "Friends, Lovers, and the Big Terrible Thing," a book that traced his origins with fame and an honest account of his highly publicized battle with alcoholism and drug addiction. His struggle with substances spanned decades.

In a statement Thursday, U.S. Drug Enforcement Administration Administrator Anne Milgram said the five defendants "played a key role in (Perry's) death by falsely prescribing, selling, or injecting the ketamine."

"Matthew Perry’s journey began with unscrupulous doctors who abused their position of trust because they saw him as a payday, to street dealers who gave him ketamine in unmarked vials," Milgram said.

In a news release and a news conference Thursday, investigators laid out the moments that led to Perry's October death at his Los Angeles home.

According to a DOJ news release issued Thursday, Plasencia, the doctor, learned that Perry was interested in obtaining ketamine in September 2023.

Later, Plasencia contacted Chavez, who used to operate a ketamine clinic, to obtain the anesthetic drug with the purpose to sell to Perry. According to investigators, Plasencia sent a text messages to Chavez that included, "I wonder how much this moron will pay" and "Lets find out."

According to investigators, on Oct. 28, 2023 — the day Perry died — his live-in assistant Iwamasa injected Perry with ketamine, as he did several times before, without proper medical licensure to complete the injections. The ketamine used in Perry's death was sold by Fleming and Sangha, officials allege, and administered with instructions and syringes provided by Plasencia.

Plasencia allegedly sold the drug to Iwamasa, although he was informed at least one week prior to Perry's death that his addiction to ketamine was "spiraling out of control." Later, Sangha allegedly texted Fleming to "delete all our messages" after news of Perry's death came out.

Later, the Los Angeles Police Department executed a search warrant at Sangha’s "stash house" and reported to have found evidence of drug trafficking, including approximately 79 vials of ketamine as well as several other drugs.

According to court records reviewed by USA TODAY, U.S. attorneys filed a complaint in March accusing Sangha of possession with intent to distribute methamphetamine. The federal case against her was updated with the first superseding indictment, which was unsealed Thursday and also named Plasencia, on Wednesday.

On Sept. 30, Iwamasa learned how to inject ketamine after Dr. Plasencia, who was nicknamed "Dr. P," administered two shots of ketamine" to Perry and then taught the personal assistant how to make injections and left behind a vial for him. For days, Iwamasa also used coded language to communicate with co-conspirators to obtain more ketamine.

But two weeks before his death, on Oct. 12, investigators say Plasencia administered "a large dose of ketamine" to Perry, which caused "an adverse medical reaction" that led to a blood pressure spike which caused Perry to "freeze up" where he "could not speak or move."

According to the plea agreement, Plasencia allegedly told Iwamasa "let's not do that again."

The court documents also appear to suggest that Iwamasa and Perry planned to take a break from ketamine use as Plasencia, who is listed as "co-conspirator 1" in court documents, texted Iwamasa: “Hi. I know you mentioned taking a break.I have been stocking up on the meanwhile. I am not sure when you guys plan to resume but in case its when im out of town this weekend I have left supplies with a nurse of mine."

On October 28, 2023, the day that Perry died, Iwamasa allegedly injected him with a shot of ketamine at 8:30 a.m. and again in the afternoon at 12:45 p.m. as Perry watched a movie. Then, about 40 minutes later, Perry asked his personal assistant to prepare his hot tub and “shoot me up with a big one," in reference to another injection of ketamine.

Iwamasa later injected Perry with a third syringe of ketamine and gave it to him "in or near the jacuzzi." According to the authorities' timetable, Perry had received three ketamine shots in a five-hour period. Later, Perry was found dead.

On Oct. 28, 2023, the Los Angeles Fire Department responded to Perry's Pacific Palisades home at 4:07 p.m. and found "an adult male unconscious in a stand-alone jacuzzi." Responding officers pronounced him dead at 4:17 p.m.

"A rapid medical assessment, sadly, revealed the man was deceased prior to first responder arrival," Nicholas Prange, an LAFD spokesperson, told USA TODAY in a statement on Oct. 30.

In December, more than a month after Perry's death, the Los Angeles County Medical Examiner's office released Perry's autopsy report, which was obtained by USA TODAY. His death was ruled an accident, with the cause being " the acute effects of ketamine ." Contributing factors were drowning, coronary artery disease and the effects of buprenorphine.

Buprenorphine is "an opioid-like drug used in the treatment of opioid addiction as well as acute and chronic pain," according to Perry's 29-page autopsy report. There were no signs of "fatal trauma and no foul play suspected," per the report.

Ketamine is a "dissociative anesthetic that has some hallucinogenic effects" and "distorts the perception of sight and sound,"  according to the DEA's website. The medical examiner said the amount of ketamine found in his system was as high as 3,540 nanograms per milliliter. "Levels for general anesthesia are typically in the 1,000-6,000 ng/ml ranges," the report notes.

There were no pills, drugs or medications found near the pool, according to his autopsy. Perry, who was reportedly sober for 19 months, didn't have alcohol or drugs such as methamphetamine, cocaine, heroin and fentanyl in his system.

Perry was "reported to be receiving ketamine infusion therapy for depression and anxiety," according to the report, and his last session was reportedly one and a half weeks before his death. But, the report notes, "the ketamine in his system at death could not be from that infusion therapy, since ketamine’s half-life is 3 to 4 hours, or less." The autopsy also said the method of intake was unclear.

"At the high levels of ketamine found in his postmortem blood specimens, the main lethal effects would be from both cardiovascular overstimulation and respiratory depression," the report reads.

"Drowning contributes due to the likelihood of submersion into the pool as he lapsed into unconsciousness; coronary artery disease contributes due to exacerbation of ketamine induced myocardial effects of the heart."

Faced once again with a rapidly spreading epidemic of mpox, the World Health Organization on Wednesday declared a global health emergency. The last time the W.H.O. made that call was in 2022, when the disease was still called monkeypox.

Ultimately the outbreak affected nearly 100,000 people worldwide, primarily gay and bisexual men, including more than 32,000 in the United States.

The W.H.O.’s decision this time was prompted by an escalating crisis of mpox concentrated in the Democratic Republic of Congo. It recently spread to a dozen other African countries. If it is not contained, the virus again may rampage all over the world, experts warned.

On Thursday, Sweden reported the first case of a deadlier form of mpox outside Africa , in a person who had traveled to the continent. “Occasional imported cases like the current one may continue to occur,” the country’s public health agency warned.

“There’s a need for concerted effort by all stakeholders, not only in Africa, but everywhere else,” Dr. Dimie Ogoina, a Nigerian scientist and chair of the W.H.O.’s mpox emergency committee, said on Wednesday.

Congo alone has reported 15,600 mpox cases and 537 deaths, most of them among children under 15, indicating that the nature of the disease and its mode of spread may have changed.
