Is there financial help for my medical bills?

Financial assistance programs, sometimes called “charity care,” provide free or discounted health care to people who need help paying their medical bills. These programs may help patients who do not have insurance and patients who have insurance but are underinsured. These programs may be provided by your medical care provider and your state. Additionally, there may be non-profit organizations or advocacy groups that can help you.

The Affordable Care Act (ACA) requires hospitals to have a written Financial Assistance Policy (FAP) and a written Emergency Medical Care policy . These policies should be widely publicized (for example, an easily seen written notice on billing statements and public displays) and must include specific information such as:

  • Eligibility criteria for financial assistance and whether the care is free or discounted.
  • Basis for calculating amounts that you are charged.
  • How you apply.

As part of the hospital intake or discharge process, the hospital must offer you a paper copy of the plain language summary of the FAP. This summary must include:

  • A brief description of the eligibility requirements and assistance offered under the FAP.
  • A brief summary of how to apply for assistance under the FAP.
  • The direct website address (or URL) and physical locations where the individual can obtain copies of the FAP and FAP application form.
  • Instructions on how the individual can obtain a free copy of the FAP and FAP application form by mail.

Steps to take for charity care or financial help with medical bills

Ask for a copy of the hospital’s financial assistance policy up front. By law, the policy must be provided free of charge and must tell you how to apply for help. You can ask whether the policy is available in your preferred language in addition to English.

2. Fill out an application form.

Fill out the application with information about your income, including last year’s tax forms or a current pay stub, and your expenses, including your rent or mortgage payment, utilities, credit cards, and other expenses.

3. Ask questions about the process.

Ask your provider how long it takes to process your application for financial assistance, how to get answers to questions about the application, and what happens with your bill in the meantime.

4. Notify any debt collectors, if they are trying to collect from you

Notify any debt collectors trying to collect on the hospital bill that you’re seeking financial assistance for the bill. You can tell them to pause collections while that is pending.

5. Follow up

Follow up with the service provider and its billing department about the status of your application as necessary.

More information about charity care or financial assistance policies is available on the IRS’s website . In exchange for nonprofit hospitals being treated favorably under federal tax laws, they are required to follow the requirements for providing help to people who need it. If you suspect the hospital is NOT following these requirements, notify the IRS .

Even if a hospital is for-profit or private, they may have a financial assistance policy that can help you pay your medical bills. Ask your hospital for information about their policy, including the eligibility requirements and how you can apply.

Some states have charity care laws that require hospitals to provide free or discounted care to patients meeting requirements, sometimes based on income. Here are some of the states that provide protections:

  • California, Connecticut, Illinois, Maine, Maryland, Nevada, New Jersey, New York, Rhode Island, and Washington have protections that apply to all hospitals .
  • Louisiana, Oregon, and Texas have protections that apply only to nonprofit or state hospitals .
  • Colorado, Massachusetts, and South Carolina have state-run financial assistance programs.

Even if your medical bill is in collection or you have been sued for the debt, you may still want to apply for charity care or financial assistance. You can also request that the debt collector stop collection activity while your application is pending with the hospital. Here is a sample letter that you can use or modify to send to the debt collector. Telling the debt collector to stop contacting you does not stop the debt collector or the hospital from using other legal ways to collect the debt from you if you owe it.

Where can I go if I need more help?

If you have problems with your hospital’s financial assistance policy or application or are unable to resolve your billing dispute to your satisfaction, you have several options.

Consumer Assistance Programs. Many states provide help for consumers experiencing problems with their health insurance. This state map will help you find assistance in your state or territory.

Centers for Medicare & Medicaid Services offers detailed information about your protections against surprise medical bills, including a help desk for medical billing questions and ways to submit a complaint if you believe your facility, provider or insurer isn’t following the rules.

State agencies such as your state attorney general and state insurance department or insurance commissioner may also offer helpful information as well as a complaint process.

Legal Services. If you need a lawyer , there may be resources to assist you, and you may qualify for free legal services through legal aid.

CFPB . If you have a problem with debt collection and a debt you don’t owe because the bill should have been or has been covered by a Financial Assistance Plan or charity care, or if you have a problem with these bills being reported on your credit report, you can submit a complaint with the CFPB online or by calling (855) 411-CFPB (2372) .

From broken arms to difficult diagnoses, any hospital visit can take you from a place of relative financial stability to struggling to make ends meet . When this happens, what are your options? How can you move forward under the weight of crippling medical debt? One of the many ways to help with that debt is a hardship letter.

A medical hardship letter is simple, yet powerful. Individuals who find themselves overwhelmed with medical debt have the option to send in a medical hardship letter. A medical hardship letter contains a plea for leniency and potential forgiveness of debts — either partial or whole. When writing a medical hardship letter, you explain to the hospital or entity controlling your debt that you are unable to pay your medical debts.

Once you have written this letter, you will send it to the hospital or entity controlling your debt with the hopes of a favorable response. But what exactly are some of the ways a hardship letter can help? Let’s take a closer look!

How Does a Medical Hardship Letter Help?

Here are some different outcomes of the medical hardship letter:

Debt Forgiveness (full)

In some cases, there may be a chance for debt forgiveness. Typically this happens in particular situations. These situations may include ones where either the debtor had been making regular payments on the debt, got it down to a small amount, and then had an unexpected financial hardship. Because the amount of unpaid debt is small, it may not be worth the time of the hospital to come after you for the debt. If you were to just try to ignore the debt, there is a chance the hospital would sell the debt to a collection agency. However, if you write a hardship letter, there is a chance that the hospital will instead opt to cancel the remaining debt.

Debt Settlement (Partial Forgiveness)

Another possible outcome is that the letter will begin the process of debt settlement . If you have debt you still owe, but have missed several payments, the hospital may consider accepting a lower payment amount for the sum of the debt. For instance, if you owe $5,000, there is a chance the hospital may accept $3,000. Once the $3,000 is paid, you would no longer owe the hospital anything.

If you were to simply ignore the fact that you were missing payments, there are many avenues of debt collection for a hospital. However, if you acknowledge the debt and explain your situation, there is a chance the hospital will instead be patient and lenient, especially if you include your plan to repay the debt.

Regardless of what you are hoping comes out of the medical hardship letter, the biggest thing this letter does is begin the communication with the hospital. As we mentioned earlier, the worst thing you can do is ignore debt that you cannot pay. Reaching out to the entity that holds your debt is the first step to obtaining a solution that works for you. So, how can you write a medical hardship letter? Here are a few things you need to include in the letter.

How to Write a Medical Hardship Letter

There are a few necessary pieces of information that will be helpful to include in this letter. Not only do you want this letter to be informative and logical, but you also want it to be persuasive and emotional. Balancing this can be difficult, but it is definitely achievable. Here are a few components to keep in mind.

Monitor the Length

While you do want enough information in the letter, you don’t want to send a novel. Be direct and to the point. The preferred length is somewhere around one full page. Too short and it may seem callous, but too long and there’s a chance the letter isn’t even read. 

Introduce Yourself

Make sure the reader knows they are dealing with a real person! Tell them your name and why you have medical debt in the first place. It could sound something like this:

Hello! My name is [insert name], and I recently was a patient at [hospital or medical office] due to [medical condition or treatment]. I was so thankful for the excellent care and service you were able to provide during such a critical and scary time.

Provide Financial Information

In this next section, you want to include the necessary financial information that lays out how much the treatment cost, what your insurance covered, and what you still owe. Keep this at the top, as this is information they need to know and are most interested in. It could read as follows:

While being treated, I incurred expenses of [dollar amount]. [Dollar amount] was covered by insurance, leaving me with [dollar amount] left to pay.  (If you don’t have insurance, you could write: Unfortunately, I did not have insurance, so I was left with the entirety of the bill.)

Previous Repayments

Call back to the times you have been paying your debt down. If you were making payments monthly, let them know! If you were making payments in chunks as you were able, point that out. Just make sure to include any effort you made to repay the debt. Here is an example:

After my treatment, I made regular, monthly payments of [monthly payment amount]. I made these payments every month for the past 7 months. 

Insert the Hardship

Now is the time to mention what is preventing you from making your monthly payments. You can be as detailed or as vague as you want, however, the more detailed you are, the better chance you have of getting a deal worked out. Whether it is an unexpected job loss, a death in the family, illness, or something else, mention what is stopping you from being able to make consistent payments. Here is something you could write:

Unfortunately, I have recently been laid off from my job and I have no meaningful source of income. Because of this, I am unable to make any payments on my debt. In fact, I am barely able to make ends meet with rent, groceries, and utilities. 

Provide Proof

If you are able, provide as much financial documentation as possible. Not only will this show that you are being honest about your situation, but this will also solidify your situation in the reader’s mind. Things like check stubs, total debt to the hospital, and even emails showing your situation are valuable to include. Mention them like this:

I have included the email that notified me of my termination of employment, as well as my previous check stubs, rent statements, and hospital debt statements. As you can see, without a meaningful source of income it is impossible for me to pay both my rent and hospital bills.

Suggest a Solution

Towards the end of the letter, suggest a solution! Whether you are able to make smaller payments, want to completely settle your debt, or need a couple of months to get situated before resuming payments, let them know what your plan is and ask if they are willing to consider it! Here are a few examples based on your desired outcome:

Fortunately, I have a savings account for times such as this. While my savings account is significant, it would not completely cover the debt in its entirety. Though my debt is $5,000, I have $3,000 I can pay immediately. If I did this, could we consider the debt fully paid? This way, I could move forward towards financial stability with one less payment to worry about, and you have one less payment you have to keep track of. 

To Lower Payment:

Though my income is significantly stalled at the moment, I would like to continue making smaller payments as a gesture of good faith. Would you be willing to accept [lower dollar amount] per month until I am able to resume working?

To Stall Payments:

I am actively working on obtaining a new job. During this time, however, I am unable to make any payments to the hospital. I estimate it will take me [number of days, weeks, months] to get a new job and resume bringing in paychecks. Would you consider allowing me to pause my payments for [amount of time] so that I can get a job and resume making payments afterward?

Provide Contact Information and a Thank You

You obviously want to end with gratitude, but also with a call to action. Leave the ball in their court and ask them to contact you with their thoughts. It could look something like this:

Thank you for your thoughtful time and consideration. I hope we are able to come to an agreement that works for both of us. Please reach out at [contact information] so we can discuss the details of this arrangement.

[name/signature]

Apply for financial assistance if you’re eligible.

Follow the policy’s instructions to apply. 

The hospital may ask you to submit a form online or print and mail a form.

Call the listed phone number if you have any questions about your application.

Check the status of your application.

After you apply, ask the hospital: 

  • How long it takes to process your application for financial assistance
  • How to get answers to questions about the application
  • What happens with your bill in the meantime

If your bill is in collections, tell any debt collectors that you're trying to get financial help.

Ask them to pause collections while that process plays out.

Writing A Financial Hardship Letter Due To Medical Bills

Medical bills tend to be substantial bills and if you incur a sudden expensive illness or a major accident, they can mount up very quickly. You may need multiple, lengthy treatments and your insurance may not cover all of those expenses. A lot of people don’t realize that credit card debt and medical debt are quite similar. Namely, they are both unsecured, which means they can both be negotiated. One way to get the ball rolling, is by writing a hardship letter.

Tips for Writing a Financial Hardship Letter Due to Medical Bills

The healthcare industry is very inflexible in terms of collecting on medical bills. However, this does not mean that you have to pay more than you can, nor do you have to take out a loan. Doing so will only make your situation worse.

First of all, therefore, you should determine whether the debt is actually correct. It is quite possible that there has been an error in your medical bill, particularly when you consider that studies have shown that up to 80% of them do indeed have some sort of error. Hence, make sure to look over the bill and ask a trusted person to do the same. You can find online help on how to read your bill as well.

Once you have determined the amount you actually owe, you need to negotiate with the medical institution to see if a repayment plan can be created. You should not have to put yourself in greater financial difficulty just to pay for your bill. This is because, if you do this, you may not be able to receive Medicaid later on in life. What you need to do is immediately tell the creditor that you cannot pay. Debt used to be referred to collection agencies after 150 days, but this is now usually between 60 and 90 days, so make sure you do things quickly. Consider that collection agencies may sue you for as little as $100, and you will see how important it is to take action now.

Make sure you keep documented evidence of everything that you do. Hence, do not speak to the medical provider – or a debt collection agency – over the telephone, but do everything in writing. You should also make payments each month on the bill that you can afford, even if an agreement has not yet been reached. This shows willingness on your part.

Some important tips for your hardship letter :

  • Keep the letter short and to the point. Try to stick to a single page.
  • Include a financial statement that shows your income and expenses.
  • Always be polite and courteous. After all, you are asking for help.
  • Explain that you are in hardship and why, and how that is linked to the medical condition in question.
  • Offer a repayment plan that you can afford, stating when you will make the payment, how, and for how long.
  • Address it to an actual individual, which gives it a personal touch.

If your repayment offer is accepted, make sure that you receive a confirmation for that in writing.

Hospital financial assistance policies were created under the Affordable Care Act.

When patients are faced with huge medical bills , it may seem like there are few avenues for relief. They can negotiate with insurance. They can haggle with the hospital. They can negotiate with debt collectors.

But a resource patients may not be aware of is hospital financial assistance, also called charity care. "It's a requirement for nonprofit hospitals to create financial assistance plans," says Jenifer Bosco, staff attorney at the National Consumer Law Center.

Understanding whether you qualify for financial help isn't always simple. You'll need to know whether the facility you visit has a financial assistance policy, the procedure you underwent qualifies and you qualify as low-income . Interested in learning more? What's a Hospital Financial Assistance Policy?

Financial assistance policies help provide free or discounted medical services to patients who meet certain eligibility standards and are unable to pay for their medical treatments. They were created under the Affordable Care Act to address medical debt and increase transparency. Specifically, these regulations apply to nonprofit hospitals and are overseen by the Internal Revenue Service, or IRS.

Patients will find that these programs run the gamut depending on the medical institution and the state in which it's located, which may have its own regulations. "There's not anything within IRS rules that says how much financial assistance needs to be provided and who qualifies for financial assistance, so the hospitals have a lot of leeway to make those decisions," Bosco says.

You may find everything from fairly generous policies offering assistance to families who earn five or six times the federal poverty guidelines – an annually adjusted measure of income and family size – or you may find that your facility offers the bare minimum. Some may offer sliding scales, wiping away more debt for lower-income patients, while others may standardize the relief offered. Nonprofit medical centers are required to make the financial assistance policy, application form and a plain language summary of the policy widely available on their websites, so you should be able to find more information about your hospital online.

How Do I Know if I Qualify for a Financial Assistance Policy?

First, you'll need to verify that the facility you visited offers financial assistance or charity care. If you received treatment at a nonprofit hospital, then federal law requires there to be a policy in place. If you received treatment at a for-profit medical center, it can't hurt to ask if there's financial assistance available, but the hospital is not federally required to offer it.

Next, you'll want to determine whether your income and family size qualify you for financial assistance. These eligibility cutoffs vary depending on the hospital. For example, the University of Pittsburgh Medical Center uses a sliding scale to determine assistance amount. If the patient earns between 400% and 251% of the federal poverty guidelines, he or she will earn partial financial assistance. If the patient earns 250% or less than the federal poverty guidelines, that patient is eligible for 100% financial assistance and for UPMC service fees to be waived. UPMC also offers hardship assistance for qualified families where medical liability exceeds 15% of family income.

Finally, you'll need to have undergone a procedure that qualifies for financial assistance. You may find that cosmetic procedures , dental care and other treatments are not covered.

How Do I Apply for a Financial Assistance Policy?

If you have questions about the financial assistance plan, where to find the financial assistance application and how to determine whether you apply, call the facility's billing office, says Maureen Lamb, CEO and founder of Medical Bill Support, which works with health care consumers to reduce medical costs. That should get you started in the right direction.

At the University of Iowa Hospitals & Clinics, the aim is to make people aware of the program when they make their appointment, says Philip Roudabush, assistant vice president of revenue cycle operations at the University of Iowa Hospitals & Clinics. They try to introduce the program to patients who are uninsured or underinsured. "The best-case scenario is when they're calling in and trying to schedule an appointment, we try to make sure they're aware of what that looks like if they don't have (an insurance) plan," Roudabush says.

But sometimes, patients don't hear about the program or are accepting emergency care and can't plan for it until after medical services are received. You'll generally need to start the application process within 240 days of receiving your first bill, Bosco says. So it behooves you to not delay.

The application may require tallying up income, sharing the names of your family members and detailing other assets. "They really vary in terms of what they might ask – sometimes they want tax forms," Lamb says.

During the application process, you can expect the debt collections process to pause. "We pause the 'dunning process' at that point because you're engaging with us," says Benjie Johnson, Michigan Medicine's chief officer of the hospital’s revenue cycle.

Generally, if you have insurance, you'll need to have exhausted all of those benefits, including employer-sponsored health insurance , automobile insurance and workers' compensation, before tapping charity care. "Our financial assistance program actually will allow for financial assistance even if you have insurance, and we made that change recently because of these high-deductible plans," Johnson says.

What Do I Do if I've Been Denied Financial Assistance?

If you don't qualify, don't despair. Hospital representatives say patients may be able to repay their bills on a low- or no-interest installment plan or take out a short-term loan with a partner bank.

You may also want to reapply in future years if you take on new medical bills. After all, your family circumstances and the federal poverty guidelines change from year to year, so a denial in the past may not exclude you from having an application accepted in the future.
